A mommy makeover, or cosmetic surgery after pregnancy, includes surgical procedures for changes in the breasts, abdomen, waist, or back following childbirth and breastfeeding. If breast sagging, abdominal laxity, or localized fat concerns you, you can describe the areas you would like to change at the examination. We explain surgical options while also considering how the breasts, abdomen, waist, and back relate to one another.

We can perform a breast lift for sagging breasts, a tummy tuck for abdominal laxity, and liposuction, or fat removal, for excess fat around the waist and back. You do not need to arrive at the consultation having decided on a particular operation. We can begin with the change that concerns you and explain which tissues could be treated.

Breast, abdominal, and waist anatomy

The breast sits in front of the chest muscle, with a skin envelope surrounding milk glands and fat tissue. In the abdomen, a fat layer lies beneath the skin, with muscles and their surrounding connective tissue deeper below. Fat distribution at the sides and the shape of the abdominal wall also contribute to the appearance of the waist. Fullness and laxity in the same area may relate to these different tissues.

An upper breast that looks empty may relate to volume, while a downward-pointing nipple may relate to the position of tissue and its skin envelope. Laxity around the belly button and fullness at the waist are also different features. The surgical plan distinguishes which changes involve skin, fat, or supporting tissues.

Suitability for surgery

Stable body weight helps with planning around the current shape of the breast and abdominal tissues. If you are considering another pregnancy, you can share this at the consultation. Pregnancy’s effects on breast volume and the abdominal wall are taken into account when deciding which procedure may be suitable.

The examination assesses breast volume and position, abdominal skin laxity, and fat distribution around the waist and back separately. For example, breast volume loss may be pronounced while excess abdominal skin is confined below the belly button. The scope of the procedure is selected according to these regional findings and general health.

Changes in the breasts or abdomen after childbirth do not, on their own, constitute a reason for surgery. Surgical options can be discussed in relation to the person’s own wish for change and how that wish relates to the existing tissues. Planning may be postponed if there is an uncontrolled medical condition, an unexplained breast finding, or ongoing substantial weight change.

You can bring information about previous cesarean deliveries, abdominal operations, or breast operations to the consultation. Previous incision locations and the distribution of current excess skin affect the boundaries of new surgical markings. If an abdominal wall hernia is suspected or a breast finding requires further investigation, its nature is clarified before proceeding with cosmetic planning.

Pregnancy plans and suitability after breastfeeding

Suitability for breast surgery requires breastfeeding to have ended and breast volume to have stabilized. While breastfeeding continues, the volume of the milk glands and breast fullness may change. Procedure selection is therefore based on breast anatomy outside breastfeeding. Regardless of the time since childbirth, the current condition of the breasts is established through examination.

Plans for another pregnancy affect the timing of surgery, particularly procedures addressing the abdominal wall and breast shape. During pregnancy, the abdominal wall expands again, and hormonal changes alter breast volume. If a pregnancy is planned soon, postponing cosmetic surgery may be considered. Sharing your pregnancy plans and current concerns brings these options into the discussion.

If weight loss is ongoing, fat distribution and excess skin around the abdomen and waist may still be changing. Planning surgery at a stable body weight allows the skin to be removed and the areas to be shaped to be identified. A mommy makeover includes procedures addressing breast volume, excess skin, and localized fat distribution. General weight loss requires a separate approach.

Changes in the breasts

Hormonal changes during pregnancy and breastfeeding enlarge the breasts. After breastfeeding, the breasts may regain their shape, remain large and sagging, or lose volume. Volume loss and sagging may also occur together in the same breast.

If sagging is the main concern, we can surgically reshape and lift the breasts. When volume has been lost, we can plan augmentation, and when sagging accompanies volume loss, we can combine augmentation with a lift. The nipple’s position relative to the fold beneath the breast is one of the features we assess when considering the need for a lift.

The right and left breasts may also differ in size. In this situation, we can reduce the larger breast or perform augmentation or lifting to different degrees on each side.

Breast augmentation can add volume with an implant or, in suitable people, fat transfer. A lift reshapes breast tissue, removes excess skin, and repositions the nipple. For a large, sagging breast, a reduction plan also includes removing breast tissue along with excess skin.

Areola width, nipple direction, and excess skin in the lower breast are considered in the markings. The areola is the colored area surrounding the nipple. An empty-looking upper breast does not, by itself, mean an implant is needed. The amount of existing breast tissue and the possibility of repositioning it help determine the need to add volume.

Abdominal skin and rectus diastasis

During pregnancy, the abdominal muscles may move apart toward the sides. When this separation persists, we call it rectus diastasis: separation at the midline of the two rectus muscles that run vertically along the front abdominal wall. We see the abdomen become more distended, particularly after meals. The abdomen may project noticeably forward, and excess skin below the belly button may extend over underwear or pants.

In a tummy tuck (abdominoplasty), we bring separated muscles closer together using an internal corset technique, meaning sutures that draw the muscles toward the midline, and remove excess sagging tissue. This addresses abdominal wall laxity and the excess skin over it.

The examination establishes how much of the abdominal prominence comes from excess skin, fat tissue, or muscle separation. These tissues require different interventions. Liposuction reduces fat tissue, while a tummy tuck addresses excess skin removal and tightening of the abdominal wall.

The rectus muscles run along the front of the abdomen from the rib cage toward the pelvis. Diastasis occurs when the midline connective tissue between these muscles widens. In surgical repair, the strong covering around the muscles is narrowed at the midline with sutures. This adjustment addresses the widened connective tissue between the muscles.

In diastasis, the midline widens; a hernia involves a specific opening in the abdominal wall. Both may occur in the same person. The examination assesses the area around the belly button and the shape of the abdominal wall during movement. When needed, imaging provides additional information about the wall’s structure.

Forward abdominal projection may reflect the structure of the abdominal wall as well as fat beneath the skin. The shape seen while standing and using the abdominal muscles helps distinguish the skin envelope from the inner supporting layer. This distinction helps establish whether midline repair should be included in a tummy tuck plan. The location of fat that can be removed through liposuction is described during the same examination.

Tummy tuck options

A mini tummy tuck is planned for limited excess skin below the belly button in suitable people. A full tummy tuck addresses laxity above and below the belly button along with the abdominal wall. An incision may also be made around the belly button. The location of excess skin and the extent of muscle separation determine technique selection.

The incision running along the lower abdomen in a tummy tuck is marked according to the skin area to be removed. If excess skin extends toward the sides, the incision plan accounts for this distribution. Cannula openings for liposuction provide access to fat tissue. The incision for excess skin removal extends along the area where the skin envelope will be adjusted.

Excess fat around the waist and back

Changes in the breasts and abdomen may be accompanied by excess fat around the waist and back. We use liposuction to remove excess fat from these areas. Liposuction is a body contouring procedure for localized excess fat.

We remove fat with a cannula, a thin, tube-shaped surgical instrument. Suction applied through the cannula draws out excess fat tissue beneath the skin.

Liposuction treats the fat layer beneath the skin. Fat surrounding organs inside the abdomen is outside the scope of this procedure. When planning treatment for waist fullness, the location of fat, elasticity of the skin envelope, and forward projection of the abdominal wall are distinguished from one another.

Folds on the back may contain excess skin as well as fat tissue. When choosing where to remove fat, the extent of the fold and elasticity of the surrounding skin are considered. Waist fullness and skin extending downward on the back may not require the same intervention. Changes in the back are therefore also described according to their own tissue structure.

How are procedures planned together?

You can describe separately which changes you would like in your breasts, abdomen, and waist. Breast volume loss, excess abdominal skin, and waist fat may require different procedures. The surgical scope is selected according to these wishes and examination findings. Seeking treatment only for the abdomen does not require a breast procedure as well.

Whether procedures can be performed in the same operation or require separate surgical plans depends on general health and the extent of the areas to be treated. A breast lift with a tummy tuck and a tummy tuck with waist liposuction are different combinations. Each combination has separate surgical limits for breast volume, abdominal wall repair, and fat removal.

Surgical plans involving breast and abdominal procedures are usually performed under general anesthesia. Previous operations, existing medical conditions, and current treatments inform the anesthesia assessment. If breast surgery is also being considered, previous breast imaging and any biopsy reports can be included in the preparation consultation. Necessary investigations relate to the scope of the selected procedures and medical history.

When a tummy tuck and waist liposuction are planned together, excess skin extending from the front of the abdomen toward the sides and fat deposits are shown in the same markings. Muscle separation in the middle of the abdomen requires intervention on internal tissues, while excess lower abdominal skin involves the outer covering. The distribution of fat to be removed at the waist is also determined according to how these front and side contours meet.

Frequently asked questions

Which procedures can a mommy makeover include?

A breast lift, tummy tuck, and liposuction, or fat removal, can be included in the plan. Depending on the breast changes, augmentation or reduction may also be options.

My breasts became smaller and sagged after breastfeeding. What can be done?

We can combine augmentation for volume loss and a lift for sagging in the same plan. Nipple position and existing breast tissue help determine which procedures would be suitable.

What is the difference between a tummy tuck and liposuction?

In a tummy tuck, we remove excess skin and bring separated abdominal muscles closer together. With liposuction, we remove localized excess fat.

What is abdominal muscle separation after childbirth?

Rectus diastasis is widening of the midline connective tissue between the rectus muscles at the front of the abdomen. In a tummy tuck plan, this area can be narrowed with sutures to bring the muscles closer together at the midline.

Does planning another pregnancy change the surgery plan?

Plans for another pregnancy are considered when deciding suitability for breast and abdominal surgery. Because these tissues can change again during pregnancy, postponing cosmetic surgery may be considered.

Is finishing breastfeeding enough to be ready for surgery?

Completing breastfeeding is one criterion for suitability. General health is also considered alongside stable breast volume and body weight when making the surgical decision.

Do I need procedures on my breasts, abdomen, and waist?

Procedures are selected according to the areas that concern you and the examination findings; not every area needs treatment. Suitability for combining the selected procedures in one operation is also assessed separately.

Where is the incision for a tummy tuck after pregnancy?

The incision is planned in the lower abdomen according to the distribution of skin to be removed. A full tummy tuck may also involve an incision around the belly button.

What type of anesthesia is used for a mommy makeover?

A mommy makeover involving breast and abdominal procedures is usually performed under general anesthesia. The anesthesia plan is based on the scope of the selected procedures and medical history.

Does a previous cesarean delivery affect the tummy tuck plan?

The location of a previous cesarean incision is considered in tummy tuck markings. How the excess skin relates to this incision and the belly button affects the boundaries of the area to be removed.

How can I begin planning a mommy makeover from abroad?

You can start with an online preliminary consultation. Sharing your photographs and the changes in your breasts, abdomen, or waist that concern you helps Op. Dr. M. Hakan Bulam offer an initial view of suitable options.

How much does a mommy makeover cost?

The cost of a mommy makeover is determined at the consultation, based on the planned procedures.
